Nevada Residents
Provide proof of residence in Nevada
The state of Nevada has legalized medical marijuana. In order for us to guide you through the process of receiving your Medical Marijuana Card, you will need to show us your Nevada Driver's License.
Eligible Medical Condition
Medical Diagnosis Required
Eligible medical conditions in the State of Nevada are: Acquired immune deficiency syndrome (AIDS), Cancer, Glaucoma, Post-traumatic stress disorder (PTSD), Cachexia (general physical wasting and malnutrition from chronic disease), Persistent muscle spasms (including multiple sclerosis), Seizures (including epilepsy), Severe nausea, Severe pain and additional conditions (subject to approval by the Division).
Doctor Recommendation / Order
Medical forms must be completed by a Medical Doctor
Health4life Medical Doctors will meet with each patient to ensure medical eligibility is met and we will complete all required medical forms for you to receive your Medical Marijuana Card.
